Note: Submitting your Forms to Pembroke Campus
Students attending the Pembroke Campus may either mail in their health forms or drop them off in person. (If submitting in person, please bring your health card as well.) If you are from Ottawa and attending a program at the Pembroke Campus, you can have your forms processed by the staff at the Ottawa Campus Health Services. Please note that blood work is not available at the Pembroke Campus, but can be done through a physicians’ office, or at the Ottawa Campus.
