Documentation Requirements

In order to receive services, students must present documentation of their disability to the CSD. For students with learning disabilities, we ask for a copy of your most recent psychological assessment, plus the Individual Education Plan (IEP) and/or IPRC. Often, these documents can be obtained from your high school resource department or school board. For students with physical, medical or other disabilities, we ask for a doctor’s letter or report that confirms the nature of the disability. The CSD has a medical form that can be provided, if necessary.
